GRAND RAPIDS, MI — A window washer fell 40 feet to his death after his safety harness malfunctioned outside the Grand Rapids Art Museum around 3 p.m. yesterday, according to the Grand Rapids Press.
Tyler Cotter was employed by Award Window Cleaning Services, a company that had a similar incident occur in April 2007, the story stated.
Grand Rapids Police Captain Jeff Hertel said: "The victim came down slightly and then fell all the way. There is no way right now to know if the harness hit him as he came down."
According to eyewitnesses, there was a commotion just before the scaffolding Cotter was standing on collapsed, causing him to fall and land on his head.
Kristin Schrewe, who witnessed the incident before calling 911, said: "He didn''t move or scream or flail his arms or anything as he came down."
Cotter, who was unresponsive to concerned onlookers who rushed to his aid, was pronounced dead at Saint Mary''s Hospital, the story noted.
Cotter and a co-worker were starting a descent down the side of the building to begin cleaning another series of windows when the tragic incident occurred, the story added.
