SAN ANTONIO, TX — Federal and state officials have opened an investigation into the falling death of a housekeeper, according to The Republic.

Gloria Rodriguez, a housekeeper at the Crockett Hotel for more than 12 years, fell six stories down an elevator shaft, where her body was found last Wednesday night, the article stated.

According to the article, police were informed that while the hotel "had been having problems with the elevators" they had all been repaired and were in working order.

State records indicate the elevator was inspected in December of 2010 and no safety concerns were noted at that time, the article noted.

The Texas Department of Licensing and Regulation, along with the U.S. Occupational Safety and Health Administration (OSHA), will be investigating the accident, the article added.
